The Hunterdon Huskies and NJAYF/NJAYC require a valid2018 Medical Release Form (certified by a physician) in order to participate in football or cheer. No participation of any kind will be allowed without the certified form. The Huskies will, once again, be offering physicals this year for a very nominal fee. The physicals will be conducted by Dr. Paul Madura, Family Practitioner. He has provided this service for us on an annual basis for a number of years now. The physicals will be offered at Union Forge Field (in the Trophy Room) on a first come, first serve basis on the following dates and times:
Cost is $15 per physical, payable directly to Dr Madura (cash or check payable to Dr Madura; no credit cards can be accepted)
Dates are as follows:
June 26th @ 6:30pm
June 28th @ 6:30pm
July 7th @ 9am
July 10th @6:30pm
For each of the days & times above, Dr Madura will be at the field for about 2 hours

Bring -- Bring a copy of the 2018 Medical Clearance Form - there may be copies at the field, but just in case, be sure to bring one with you!
Note - you only need to go to one of the dates above, and only if you are unable to get the attached 2018 Medical Clearance form signed & stamped by your own physician with a 2018 date.
This form is required for all Huskies athletes, excluding Huskies Minis Cheer.
