Hunterdon Huskies Fall Football and Cheer Required Documents


Please see below for the documentation requirements for all Huskies football and cheer participants. Please note you will be provided with additional instructions from either your team coach or from the football/cheer coordinator, with regards to documentation collection and due dates.

Please note that, per our national rulebook, athletes are not allowed to practice without these completed documents


2023 Required Documentation for All Participants (all forms are also available on the Downloadable Forms tab) 

  1. AYF Medical Clearance Form: Must be signed, stamped and dated (with a 2023 date) by physician [this form is not required for Minis Cheer].
  2. AYF Emergency Medical Information Form: To be completed by parent - please complete all fields.
  3. AYF Player Tracking Participation Card: To be completed by parent - please complete and initial all fields.
  4. AYF Release of Liability: Signed by parent and player
  5. AYF Image Release
  6. AYF Annual Concussion Acknowledgement
  7. Birth Certificate: Copy of birth certificate [for Cheer – only if new to Huskies this year; please show the original to cheer coordinatori].
  8. Final 2022-2023 School Report Card: Copy both sides, make sure the photocopy does not cut off any information
  9. Work Bond Check: $200 payable to HBAA, no date, per family. This is not cashed, and will be destroyed at the end of our season, so long as your volunteer requirements are satisfied prior to December 2023. Must be handed in with documentation.
  10. Uniform Safety Deposit: $225 payable to HBAA, no date, per participant. This is not cashed, and will be destroyed at the end of our season, so long as you return your Huskies-issued uniform prior to December 2023. Must be handed in with documentation.
